Dear Parents / Guardians;
Welcome to Wareham Public Schools. We are excited to welcome your child into our school community of learners. Listed below are the forms and documents needed for registration. Please call 508-291-3500 extension 3541 for an appointment as we are looking forward to enrolling your child.
The following documents are required for registration:
- Original birth certificate
- Proof of residency (see Residency Policy)
- Immunization records and recent physical (within 1 year)
- Proof of guardianship (if applicable)
- Transcripts and up to date grades
- MCAS/SAT scores (if applicable)
- Attendance and discipline records
- IEP or 504 (if applicable)
- Completed registration packet (links below)
*Students will be enrolled after all documents have been received by the Registrar.

Preschool Program Information
Wareham Public School's Integrated Preschool programs include children with and without special needs. Children without diagnosed learning needs are considered Peer Models. An application is required for all peer models. All peer models must be toilet trained in order to enroll in our program. Children must be 3 years old by September 1st of the school year they are applying for.
Due to the large number of applicants and limited number of openings, we give preference to children who are 4 years old. If your child is not chosen, they will be put on a waiting list.

The School will notify the student when all forms have been entered and when the student should report for their first day.
